A violation report can be submitted by the Whistleblower in writing, by post, in an envelope marked with “violation report”.
It should be sent to the address of SEEN Technologie Sp. z o.o., ul. Dziekońskiego 3, 00-728 Warsaw.
For the report to be effectively considered, the Whistleblower should provide the most detailed description of the event and its context.
The report should include the following information:
- Identification of the person or organizational unit (team, department) in the Company to which the report pertains.
- Description of the violation, along with an approximate period in which it occurred.
- Identification of individuals who may have knowledge of the violation.
- Any other documents or evidence related to the subject of the report that may assist in a thorough review of the matter.
- Whistleblower's details - in the case of anonymous reports, this point does not apply.
- Preferred form of contact with the Whistleblower, ensuring the confidentiality of the report. This also applies to anonymous reports.
Anonymous reports will not be accepted or considered.
The Whistleblower should provide their contact details in every case to enable effective verification of the report.
4. Confirmation of report receipt
Upon receiving the report, the Whistleblower will receive confirmation of its receipt within 7 working days.
The confirmation will be sent to the contact address provided by the Whistleblower.
